Patient Profile: Burmese-speaking patient with limited engagement history and A1c of 13.8%

Medications: 

  • Trulicity
  • Metformin ER
  • Atorvastatin

Measures Taken:

  • RN and clinic collaborated to connect with patient
  • Discontinued Lantus (brand name Insulin injection) after improvement


Clinical Results:

  • A1c trend: 5.5% Reduction in 3 months
    • In December 2023, the patient engaged with the clinic and enrolled in the program with an A1c of 13.8%.  Throughout her engagement, she has reduced her A1c to 5.3% and was able to discontinue insulin therapy.
  • Patient now has normal A1c
  • BP controlled without medication
